Description
MOC's CARE AIDS Services Project provides a wide range of free services to individuals living with HIV/AIDS and their family members. Services are aimed at increasing engagement in medical care and include: medical case management; nutrition related programming and food pantry services; medication/medical equipment purchasing assistance; medical transportation; peer support programming; housing search, permanent supportive housing, and financial assistance to prevent homelessness. Program eligibility is limited to individuals who are HIV positive and their family members. Documentation of HIV status must be provided within one month of intake and should be either a letter from a medical provider or lab results.
